Juventus Proof of Vaccination Policy

Juventus Cycling Club has implemented a proof of vaccination policy for vaccine eligible persons effective December 1, 2021. Proof of full COVID-19 vaccination OR proper documentation of a medical exemption (signed by a Nurse Practitioner or Physician) is required for participation in Juventus programming and group activities.

 Members with unique circumstances may apply in writing to the board to request an exception. We are adopting this policy and process to safeguard the health of our club members, their families and the community at large.

 In making this decision, the executive leadership team reviewed recommendations from Alberta Health Services. An acknowledgement of vaccination OR properly documented medical exemption will be included on 2022 Zone 4 Juventus membership. The Juventus Board reviews our vaccination policy regularly and as of February 13, 2022, has decided the policy will remain in effect. It will next be reviewed March 15th 2022.

Please be prepared to produce your proof of vaccination or documentation of medical exemption for Juventus programming or group activity commencing after December 1, 2021. Your COVID-19 Vaccine Record can be found here:  https://covidrecords.alberta.ca/home.
